As part of the medical review process, the MOD-affiliated provider will need to verify your identity. In order to confirm your identity, you will be asked to upload your U.S. government-issued ID.

If you are having trouble uploading your ID, please send an email with an attachment of your ID to support@mod.com, and the support team will be happy to upload it to your account for you.

Yes. All patients are required to complete a live video chat with a MOD-affiliated medical provider as part of their digital consultation. After checkout, our system will automatically prompt you to begin your video consultation.

As with any in-person medical provider in a hospital or clinic, you are responsible as a patient for providing accurate and honest answers regarding your health, medical history, and any medications you take. This ensures that our MOD-affiliated medical providers can make the best decision about whether it is safe for you to take our compounded medications.

If you are approved for the medication, you must read the instructions and safety information before taking it. These instructions are shipped with each subscription order and are also sent via email.
